Your health insurance card will be mailed to you within the first two weeks of each
new quarter. If you don't receive your card, come to the ISP office with your student
ID card to request a new insurance card.
Before you travel outside the United States, you must visit the ISP office to get
a travel approval signature on your I-20 Form.
Download the F-1 Document Request Form, which you'll find on the ISP Student Forms webpage.
Fill out the form and submit it to the ISP front desk at least two weeks before you
plan to travel. (If you submit the form later than two weeks before traveling, you
may be asked to fill out a credit card payment form for a $75 express mail fee, so
we can mail your I-20 to your destination.)
Bring a photo ID with you to pick you your I-20 form two or three days before you
plan to travel.
Remember there are hundreds of requests to travel during winter break and summer,
so it's best to submit the form as soon as you can, to make sure it's ready in time
for your departure.
If you are completing your studies or decide to leave De Anza College at any time,
you must notify the ISP office by submitting an Exit Form. You'll find the Exit Form
on the ISP Student Forms webpage.
If you are transferring to another school, follow the steps listed on the Transfer Out Checklist to make sure your SEVIS record is properly transferred to your new school. Exit
Workshops are offered each quarter to help students with the exit process.
Check MyPortal to make sure you don't have outstanding fees on your account. Accounts
left with holds, unpaid tuition or unpaid health insurance fees will affect your ability
to return to De Anza or to request transcripts and receive your degree. They may also
affect your personal credit rating or financial score in the United States.
