Students and employees must submit proof of COVID-19 vaccination and booster before
coming to campus. Proof of vaccination and booster must be submitted separately. See instructions for coming to campus.
Fully vaccinated means you must have received your final shot – your second shot for the Pfizer or Moderna vaccine, or your first shot if it's
the Johnson & Johnson (Janssen) vaccine – at least two weeks before coming to campus.
We understand if you have questions or concerns. De Anza is following the advice of
local, state and federal officals who all agree that the COVID-19 vaccines are safe and effective. You can learn more at this California Department of Public Health website.
You may request an exemption from the vaccination requirement on the basis of verified medical reasons or sincerely held religious beliefs. You'll find instructions and forms for submitting a request on
the Vaccination Exemption webpage.
We've made it easy to upload a photo or electronic copy of your vaccination card by using the PyraMED online health portal, a secure medical records system that you can access through the Apps section in MyPortal.
You should be able to log in with your eight-digit Campuswide ID (also known as CWID)
and your MyPortal password. Contact the Student Health Services office if you are unable to log in.
If you are a new student, please allow one day for your student account to be added to the PyraMED health
system, before you try to log in. You should be able to log in once you have been
assigned a registration date, which will be listed in MyPortal
If you are a high school student seeking dual enrollment at De Anza, you won't receive a registration date – or be
able to log in to the PyraMED health system – until you have submitted your High School
