CIS Prerequisite Clearance Form Processing for Summer 2021 begins May 10th.
As of July 1, 2012 all students enrolling in CIS classes must have taken the designated prerequisites for the course(s) they wish to enroll in. Students who have not taken the designated De Anza CIS classes may submit a Pre-Requisite Clearance Form which details which course they wish to enroll in and what they have completed previously to meet the prerequisite requirements. The form along with supporting documents must be submitted to the CIS Department Coordinator at CISclearPrerequisites@deanza.edu.
The following documents may be downloaded and edited using either Microsoft Word 2003 or later. The only types of documents accepted are Word, pdf, and jpeg. If you have any problems with these documents, please contact Mary Pape.
Attention Transfer Students
Check with a counselor before applying for a prerequisite clearance. Even if De Anza College clears a prerequisite it does not mean that the college you are transferring to will grant credit for the prerequisite. This also applies to prerequisites cleared using AP Exam scores.
