U.S. High School Transcript Assessment Form

When you completed the CCCApply application, you provided De Anza with your U.S. high school transcript information. Most students earn their English or ESL and math course placements based on the assessment of information.

Visit MyPortal > Apps > Student Registration tile and under 'Student Records,' click 'View My Placement Results' to review your course placements and/or next steps.

If you need to submit a U.S. High School Transcript Assessment Form to the Assessment Center, here is what you should do

  1. This completed form 
  2. Email it to daassessment@deanza.edu

Per Federal law, we are required to obtain your written approval to release your official records.

2006 California Chemistry Diagnostic Exam Score Transfer Form

A score of 22 or higher out of 44 on the 2006 California Chemistry Diagnostic Exam is required to earn placement into CHEM1A and BIO6A.

What you should submit:

  1. This completed form
  2. Email it to daassessment@deanza.edu 

Note: We can only accept scores earned from the 2006 California Chemistry Diagnostic Exam. We cannot accept any other scores, results or clearances earned from other sources.
