Faculty Profile Request Form

This Request Is For The Old Design Faculty Profile

To request a faculty site in the new design that will roll out on May 1, 2018, use this other Faculty Site Request Form .

You will be able to edit your new design site prior to May 1, but it will not be available to students and the public until the May 1 rollout of the entire new website.

Sites created in the old design will be copied over into the new design sometime before May 1. An instructor can then edit the new design site during the time leading up to the rollout to keep the information current and matching to the old design site.

If you can wait until May 1 for your faculty site to be available to the public, then please request a faculty site in the new design

Empty Fields: If you choose to not fill in one of the non-required fields (your biography, as an example), then that subheading will not appear on your faculty profile page.

Go to the Searchable Schedule, choose a quarter term, and enter your name into the Instructor field near the bottom of the page, then do the search. If a different spelling from the above is needed for the search to work, please enter here the alternate spelling that does work. Otherwise leave blank. ( Elizabeth Ross, Robert DeHart )
Enter the name of your main department.
Select the color (See options).
Enter words or phrases that will optimize your profile/website for search engines. Separate the words by a comma. Don't add generic keywords such as 'De Anza College'. Good keywords are the names of classes you teach and terms related to the material. ( digital photography, digital camera, photography )
Enter a favorite quote or a short greeting to your visitors.

Choose your classification. If your classification is not listed, please enter your classification in the Notes to Web Team.
Enter your De Anza phone number or preferred phone contact information.
Enter your "lastnamefirstname@deanza.edu" email address.
Please confirm your email address.
Enter your office location, or, if you have no office, the name of the building of your department's main office.
Enter your office hours. Will be listed by appointment as default.
Describe your areas of focus and specialization.
