Classified Professional Employee of
the Month Nomination Form
A nominee must be a permanent classified or confidential employee at De Anza College.
- Nominations are due on the first day of the month.
- Employees may be nominated one or more times but may receive the award once.
- Nominations will be retained for one year from the month that it was submitted.
- An employee's supervisor or other management, classified or confidential staff, faculty and/or students may make a nomination.
- The awards will be presented once a quarter.
