How to Apply to Receive Accommodations and Services
You must be a De Anza College student with a Campus Wide Identification (CWID) number and personal password.

THE APPLICATION PROCESS FOR NEW AND RETURNING* STUDENTS:
The De Anza College DSPS division uses the "CLOCKWORK" online software system for students to apply for accommodations and services. Follow the steps listed at the bottom of this page to access the online application. Upon completion of the application, the student will meet with a DSS Counselor or LD Specialist to set-up their accommodations and services.
During the online DSPS application process, you will be asked to upload your most recent disability related documents. Documentation examples:

*Returning students are those who have not been enrolled at De Anza for at least one year.

STEPS TO ACCESS THE APPLICATION FOR SERVICES:
Go to the De Anza College MyPortal and log in using your CWID number and password.
Select the "Student" tab then locate the "Clockwork Student Portlet" box (bottom of the page)
Select "ClockWork De Anza Disability Progs & Svcs"
On the "CLOCKWORK" main menu, click on the "DSPS Application" icon
Read the instructions, then select the "Register" or "Click Here" button to begin filling-out the application.
WHAT HAPPENS NEXT?
Upon completing the application and clicking the "Submit" button, a DSS staff member will promptly contact you to set-up an appointment with a DSS Counselor or Learning Disabilities Specialist.
