How to Apply as a De Anza College APE Student 

New and Returning Students 
In order to receive your disability-related services and accommodations at De Anza College, you must first complete these steps:

Apply to De Anza College

De Anza uses the statewide application system known as OpenCCC. This online system is designed to accommodate most students.


Complete the De Anza College application and you will receive your 8 digit campuswide ID (CWID) number and password to access the college’s MyPortal site.

Important Information

When you fill out the application, remember to declare a major (not "undecided") and select a goal of transfer, degree or certificate if you want to qualify for priority enrollment.

Submit DSPS Application

Complete and submit the online DSPS Application for services:

Login to MyPortal

  1. Find the Apps menus and clock on the Students tab.
  2. Clock on the ClockWork icon and select De Anza.
  3. Click on the DSPS Application icon.
  4. Complete and submit the DSPS Application.

If this process is unavailable, please follow the option for submitting the Disability Verification Form.

Submit Disability Verification Form

Submit a completed Disability Verification Form to verify your disability. 


Give your Doctor or appropriate health professional the following documentation:

  1. Instruction Letter
  2. Disability Verification Form
  3. Disability Definitions and Documentation sheet

Submit the completed Disability Verification Form to Disability Support Services by:

  • FAX to (408) 864-5492
  • EMAIL scanned attachment to
  • IN PERSON to RSS 141

See DSS Counselor

Complete these steps and you will be promptly scheduled to meet with a DSPS Counselor to set up all of your classes, services, and accommodations.

How to Continue as a De Anza College APE Student 

Current APE Students
In order to continue APE enrollment, students must complete a registration packet that is available at PE-13, Pool Shed, and DSPS Division office during the registration period.

Below, you'll find instructions on how to complete the registration packet.

Registration Form

Check the box next to the courses you are requesting.
Provide the following: 

  • Student name and 8 digit campuswide ID (CWID) number
  • Select your courses

Important Information

Make sure you do not register for the same course section ID.

Example: PEA D004X-01 and PEA D004X-02, has the same 4X course ID.  

Course Repetitions Form

Complete a course repetitions form for each individual course.
Provide the following: 

  • Student name and 8 digit campuswide ID (CWID) number
  • Course ID number & section, CRN number, quarter & year
  • Circle A, B, or C for the reason you are repeating the courses. 
  • Student signature


Refer to the back of the registration form for the quarter enrollement fees.  

Check or Money Order
Posted dated checks will not be accepted. 

Checks and money orders are made out to De Anza College. 
Provide the following: 

  • Student CWID number on the memo

Important Information

Credit cards will no longer be collected and processed with the registration packet. Complete the credit card prepay form on the back of the receipt and present it to the Cashier's office for further processing. Keep the receipt/credit card prepay form for your records.  


Make sure that all forms are completed before submitting the registration packet.

  • Keep receipt

The registration packet is to be submitted to one of the following areas:

  • Lockbox at PE-13
  • Lockbox at Pool Shed
  • DSPS Division office AT-209

Important Information

Please make sure to place your registration packet and payment in a sealed envelope. Your payment should be stapled to the paperwork.
